Parent Information Checklist

The following information is required before your son or daughter may enroll in Stafford County Public Schools:

Physical Exam
Complete Immunization Record

          DPT, DtaP, DT, or Td - Minimum 3 doses with one after 4th birthday
          OPV or IPV - Minimum of 3 doses with one after 4th birthday
          MMR - Minimum 2 doses 1st one after 1st birthday
          Hepatitis B - 3 doses
          Varicella (chicken pox vaccine)

Proof of Residence
Rent or Mortgage Statement
Utility Bill
Stafford County Motor Vehicle Registration

Birth Certificate
If not available, affidavit must be completed, signed, and notarized
